On most cases it is secured to the outside of the tank as some can get pretty hot. I have a tank with heat mat inside but the heat mat never gets too hot.

You fix it to the side, by putting a thick layer (at least 6mm) of polystyrene/styrofoam on the back to force the heat one way. Just tape it on, or hold it against it by putting it against the wall.
